The "Personal Information for New Hires/Rehires" form will ensure that your address, emergency contacts, and all other personal information are captured in Dow's personnel system. Please print legibly so that your record is accurately represented.
All information is required with the exception of the area entitled "Voluntary Information."
Please provide your current address on the form. If you are changing addresses due to relocation for your new position and you have not yet established your new address, you can update/change your permanent address at a later date via the Dow Intranet.
Your paycheck or pay stub (if you have direct deposit) will be mailed to your home address.
Please ensure that you fill out the emergency contact information completely. (This information was instrumental in ensuring that Dow employees were located after hurricanes Katrina and Rita hit the Gulf Coast.)
